Equipment Blamed for Release of Galveston Wastewater Sludge |

Experts blame equipment failure for the release of about 135,000 gallons of wastewater sludge into Galveston Bay. Galveston officials say Tuesdayâ??s accidental release from the $75 million main wastewater treatment facility should have no impact on the cityâ??s drinking water supply. A city statement says the incident involves partially aerated sludge.

Equifax: Hackers Exploited Flaw Experts Flagged in March |

Security workers discovered, and created a fix for, the vulnerability that allowed attackers into the Equifax network two months before the company was hit by hackers. Equifax told USA TODAY late Wednesday that the criminals who potentially gained access to the personal data of up to 143 million Americans had exploited a website application vulnerability known as Apache Struts CVE-2017-5638.
The fix for that flaw was first released March 10, though it was later modified, according to the National Vulnerability Database. Equifax said that the unauthorized access began in mid-May. That's a period of two months in which the company could have, and should have, say experts, dealt with the problem.

Eight Dead From Sweltering Nursing Home as Florida Struggles After Irma |

State officials, utility executives and the Rehabilitation Center spent Wednesday trading blame over why and how its patients were left to endure such conditions, even though state and federal regulations require nursing home residents to be evacuated if it gets too hot inside. The Hollywood Police Department opened a criminal investigation into the deaths of the eight residents, who ranged in age from 71 to 99, and investigators from the state attorney generalâ??s office were also involved. Gov. Rick Scott ordered a moratorium on admissions at the nursing home. By dayâ??s end, the unanswered questions were still outstanding, even as the deaths magnified scrutiny on other facilities for the old and disabled.

Texas Justices Query Limits Of School Sex Harassment Claim |

The Texas Supreme Court weighed a school district's challenge Wednesday to a former gym teacherâ??s sexual harassment and retaliation claims, questioning whether the allegedly harassing comments should be treated as mere bullying. Catherine Clark, who taught physical education at the Alamo Heights Independent School District in San Antonio, is alleging that she was fired in retaliation for complaining about years of harassment by her direct supervisor and another female coach, who allegedly made a series of sexually charged comments to and about Clark.

$57M Award to Pennsylvania Woman in Pelvic Mesh Suit |

A subsidiary of Johnson & Johnson has been ordered to pay $57.1 million in damages to a Pennsylvania woman in a pelvic mesh lawsuit. The lawsuit alleged that the woman was left chronically incontinent and in constant pain after receiving defective pelvic mesh implants. On Thursday, a Philadelphia jury ordered Ethicon to pay the large sum to the 51-year-old woman from York County. The award is the fifth and largest to date verdict in the lawsuit against Ethicon over pelvic mesh injuries.

Target Recalls Nearly 180,000 Dressers That Can Tip Over |
|
Target says it is recalling nearly 180,000 dressers because they can tip over and pose a risk to children. The retailer said Wednesday that it has received 12 reports of its Room Essentials four-drawer dressers falling over or collapsing. Two 3-year-old children were struck by a dresser, but no injuries have been reported. The U.S. Consumer Product Safety Commission says customers should stop using the $118 dresser immediately and return it to any Target store for a full refund.
