Despite FDA warnings and settlements, nursing homes still push anti-psychotics. The U.S. Food and Drug Administration never approved Risperdal to treat symptoms of dementia. In fact, the FDA had issued a black box warning =E2=80=93 its highest level =E2=80=93 linking the drug to an increased risk of death among elderly dementia patients. Still, doctors in nursing homes across Texas continue to prescribe the drug to dementia patients to calm them down.

Lawsuit Alleges Anadarko Cut Safety Budget Before Colorado Blast

A shareholder lawsuit alleges Anadarko Petroleum was focused on keeping oil and gas flowing from older wells, not fixing potential safety problems, in the months before a fatal house explosion in Colorado linked to an Anadarko well. The lawsuit cites former company employees as saying Anadarko had slashed its safety budget and staff and had only a skeleton crew for refurbishing wells.

Tesla Crash that Killed Two Florida Teens Probed by NTSB investigators
The National Transportation Safety Board said Wednesday that it was sending four investigators to probe a Tesla Model S crash that killed two Florida teens when it hit a concrete wall and burst into flames. Two Fort Lauderdale high school students were killed in the crash, while a third 18-year-old was ejected from the vehicle and taken to a local hospital, according to Miami's NBC 6. The decision to a launch an investigation centers on the fire that erupted after the battery-powered sedan hit the wall.

Seattle to Face Ride the Ducks Lawsuit

The city of Seattle will be facing a lawsuit over a Ride the Ducks excursion which ended in the deaths of five people and injuries to 69 others. The central question of the lawsuit is who is responsible for building a median barrier on the Aurora Avenue Bridge, which some say could have prevented the crash. The city argued that it is not responsible for building the median, but a judge has left it up to a jury. The first trial in the lawsuit is scheduled for October.

Grand Jury Indicts Two Men After Racing Crash Kills Two

Two Texas men have been indicted for killing two children while racing. The men were racing cars in Cedar Park when they hit the children, killing them. According to the Williamson County District Clerk's office, the men were recently indicted on two counts of racing on a highway and causing death and one count of racing on a highway and causing serious bodily injury. Along with the deaths of the children, one woman was injured in the crash. The men are also facing a lawsuit by the father of one of the children killed.
