Report: Safety Deficiencies Led to 2014 Chemical Release Killing 4 at DuPontâ??s

The U.S.
Chemical Safety Board on Tuesday issued its final report on a deadly release
of toxic chemicals at DuPontâ??s LaPorte facility in 2014, concluding that a
long chain of failures, including in equipment design and lax safety, led to
the deaths of four workers. Interim Executive Authority Kristen Kulinowski
summed up the findings: â??This is a textbook example of the catastrophic
consequences when process safety management is inadequately implemented and
monitored.â??

Spot: Miners Died As Bosses Refused Safety Equipment |

A Center
for Public Integrity review of MSHA investigative reports, police files and
court documents reveals that weak oversight has mixed with mistakes at mines
to deadly effect, as the industry and its regulators bicker over proposed
rules. Various types of heavy machinery have directly or indirectly been
involved in nearly 500 deaths, dozens of them caused by blind spots, at
underground and surface mines since 2000, according to MSHA data.

Anderson Patient Died After Receiving Contaminated Blood |
|
The
report, issued Monday by the Centers for Medicare and Medicaid Services, notes
the patient, a 23-year-old female leukemia patient, died two days after
receiving a transfusion tainted with a bacterial infection commonly acquired
in hospitals but rarely found in blood transfusions. She had had serious
complications prior to the transfusion, the report noted.

the dangers, riders are exposing themselves to liability and are most likely
not insured for the damages they may cause.A rider's personal health insurance
â?? if he or she has it â?? could help defray the cost of their own medical bills
in case of an accident. But it's another matter entirely when a scooter rider
hits and injures a pedestrian, damages someone's property or causes a car
accident. The rider may be held responsible, and most insurance policies will
not cover those expenses.
