Fed Report: Safety Deficiencies Led to 2014 Chemical Release Killing 4 at DuPontâ??s La Porte Plant
The U.S. Chemical Safety Board on Tuesday issued its final report on a deadly release of toxic chemicals at DuPontâ??s LaPorte facility in 2014, concluding that a long chain of failures, including in equipment design and lax safety, led to the deaths of four workers. Interim Executive Authority Kristen Kulinowski summed up the findings: â??This is a textbook example of the catastrophic consequences when process safety management is inadequately implemented and monitored.â??

Blind Spot: Miners Died As Bosses Refused Safety Equipment

A Center for Public Integrity review of MSHA investigative reports, police files and court documents reveals that weak oversight has mixed with mistakes at mines to deadly effect, as the industry and its regulators bicker over proposed rules. Various types of heavy machinery have directly or indirectly been involved in nearly 500 deaths, dozens of them caused by blind spots, at underground and surface mines since 2000, according to MSHA data.

6 New Alleged Victims of Dallas Senior Living Serial Murder Suspect Named in Suit, Bringing Total to 18

Billy Chemirmir, 46, was already accused of killing 12 women in Dallas and Collin counties. The lawsuit suggests five more women and one man were also killed. Family members filed the suit Tuesday against The Tradition-Prestonwood, a senior living complex where eight of the people died. The relatives allege the complex failed to protect residents and tried to hide a string of suspicious deaths â?? even after Chemirmir was arrested.

MD Anderson Patient Died After Receiving Contaminated Blood

The report, issued Monday by the Centers for Medicare and Medicaid Services, notes the patient, a 23-year-old female leukemia patient, died two days after receiving a transfusion tainted with a bacterial infection commonly acquired in hospitals but rarely found in blood transfusions. She had had serious complications prior to the transfusion, the report noted.
