Slack Management Exposed BP to High Safety Risk: Leaked Report |
A report, co-authored by BP, IBM and industry consultancy WorleyParsons, states that the British company's refining and petrochemical business, known as downstream, is trailing rivals such as Royal Dutch Shell by up to seven years in managing information to reduce safety risks and financial losses. "Inadequate management and use of engineering information has been a root cause or contributing factor" in 15 percent of 500 high-risk incidents reviewed in the report, which was provided by Greenpeace. http://www.reuters.com/article/us-bp-safety-idUSKBN1421UQ?feedType=RSS&feedName=topNews&utm_source=twitter&utm_medium=Social, Reuters 12/13/2016 |

4M Americans Could be Drinking Toxic Water and Never Know |
Millions of Americans face risks because the nation's drinking-water enforcement system doesn't make small utilities play by the same safety rules as everyone else, a USA TODAY Network investigation has found. Tiny utilities - those serving only a few thousand people or less - don't have to treat water to prevent lead contamination until after lead is found. Even when they skip safety tests or fail to treat water after they find lead, federal and state regulators often do not force them to comply with the law. Data Tools Offer Hints at How Judges Might Rule |
Lawyers looking for an edge in court are increasingly turning to hard data to predict how judges might rule, in some cases long before the judges put pen to paper. New tools, mined from millions of court documents, offer lawyers statistics on the likelihood of a lawsuit's being dismissed, for instance, or the average wait time until a trial. Lawyers say the data can help temper client expectations, influence courtroom decision-making and even save money by flagging strategies unlikely to succeed. $3.9M Wrongful Death Lawsuit Filed Against Oregon Tavern |
A $3.9 million wrongful death lawsuit has been filed against an Oregon tavern for over-serving a woman who was involved in a fatal crash. The woman who had been drinking at the Keg Tavern in Eugene left the bar and fatally struck a pedestrian in her car. The wrongful death lawsuit was filed by the man's son and alleges that the bar over-served the woman prior to the accident. The lawsuit, filed in Lane County Circuit Court, names as defendants the tavern and the driver, who was sentenced to nearly eight years in prison after pleading guilty to several charges in January.
